Welcome to the registration process at Vitality Health & Wellness. Thank you for choosing us as your health care provider.   We hope to make this process as easy as possible. Currently, just our "New Patient Form" can be submitted on-line. All other forms need to be printed, filled out and signed prior to being mailed or faxed to us at (305) 466-1160.  Please keep a copy of the filled out form for your records. If you have any problems downloading these forms, please contact our office at (305) 466-1100 and they can be faxed or mailed to you. 

In order to make the most of your appointment, these forms must be completely filled out and arrive at our office one week prior to your initial appointment or phone consultation.  We will attempt to contact you if we have not received the forms, however, your appointment will be cancelled and you will will be placed back on the waiting list if we do not receive them.  So please, take some time and fill them out right now.  

Many patients, especially those in other states or countries, meet with Dr. Levinson via phone consultation. We ask that patients email us a "head shot" photo (if the patient is a child, then a picture of the parent and their child is acceptable). Please send the photo to info@vitalitywellness.com with the patient's name in the subject heading.

 
In order to treat the whole person and/or address chronic ailments, the level and type of care given at Vitality is different than what is commonly available; it requires more time and attention than the standard of care that is currently offered by most other physicians.

In order to provide the level of clinical care that Dr. Levinson believes is necessary, it is not possible for us to accept assignment of payment from your insurance company.  Additionally, we do not participate as an “in-network” provider on any HMO or POS insurance plan, thus we are a “fee-for-service” clinic.  We do appreciate that this is inconvenient and might be a financial burden, however, we believe this is the best investment for your family’s health that you can make and are confident it will be worthwhile. 

We will work with you to provide bills acceptable to your insurance carrier for reimbursement and to utilize your insurance for in-network testing when appropriate.  However, there are tests that may not be available through your “in-network” labs such as Quest Diagnostics, Unilab, and Labcorp.  For the alternative “out of network” labs that he uses, Dr. Levinson has negotiated very competitive rates on behalf of his patients. 

Payment is required at the time of your consultation unless other arrangements have been made prior to the appointment.  For your convenience we accept Visa, MC, AMEX, cash and checks. 

 

Dr. Levinson sets aside a significant portion of the day for each appointment.  We never “double book” patients into the same time slot.  Therefore, it is necessary to enforce a strict 24 hour cancellation policy during the week.  For appointments following a weekend or holiday, appointments must be cancelled by one p.m. on the “work day” prior to the appointment.  The only exception to this policy is a medical emergency.

For outreach clinics outside of Miami, the appointment is charged at the time the appointment is scheduled and refunds will be made only if someone on the waiting list can take that time slot. 

Cancellation fees are equal to the standard fee for the scheduled appointment.   For those appointments canceled with appropriate notice, our staff will always give out a cancellation code for your records.

 
The vast majority of patients use Dr. Levinson for full service care.  However, if you are currently working with a physician that you are comfortable using for some aspects of your medical care, Dr. Levinson is happy to work in conjunction with them to any degree that you wish. 
 
To get the most out of the first appointment...

  1. Parents of affected children should have completed the appropriate intake form submitted online Neurodevelopmental Delay Intake Form.
  2. Fill out hard copies of the downloaded "PDF" forms below. 
  3. Fax or email relevant lab tests done by other physicians.
  4. Email a picture as described above.
  5. Email a list of current supplements including quantity of supplement and amounts of ingredients contained in it (not necessary for Kirkman Labs).
